I hereby give consent for my child to go on this field trip and I will accept responsibility for any damage or injury caused by my child to persons or property of said hotel.

I give my permission for my child to be given emergency medical aid if necessary. I understand that any medical or hospital costs incurred for my child shall be my responsibility.

I understand that I have signed an SMCAA waiver and am deemed to have waived all claims against SMCAA for injury, accident, illness, or death occurring during or by reason of the field trip.
